REQUIRED POLICIES AND AGREEMENTS
Assumption of Risk & Release of Liability
I/My child (if under 18), plan(s) to participate in programming that will take place Prima Ballet School.
I/we understand that dance training requires sustained, repetitive, vigorous physical activity. I/we understand that participants engage in a broad range of movements, including bending, twisting, running, leaping and lifting, which place extreme demands on the human body, including stress of joints and ligaments, repetitive impact, and occasional falls, slips and collisions with other participants and objects. I/we also understand that dance instruction involves kinetic corrections that may include physically touching the student as part of regular class work or rehearsals.
I/we understand that, despite precautions, there are inherent risks of serious personal injury involved in these activities. By signing this release form, I/we acknowledge and assume all risks of participation in programming taking place at Prima Ballet School and rented community spaces.
I/we agree to release, indemnify and hold harmless Prima Ballet School including its teachers, dancers, staff and members from any liability, damages, causes of action, claims, or demands, now and in the future, that may arise as a result of my/my party’s participation in programs held in Prima Ballet School's studios or rented community spaces. I/we will not hold Prima Ballet School liable for any personal injury or any property damage caused in whole or in part by my participation in programs held in Prima Ballet School’s studios or rented community spaces.
I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND I AGREE TO BE BOUND BY IT.
Medical Emergencies
In case of physical or medical emergency, I hereby authorize Prima Ballet School to make necessary arrangements to transport myself or my child to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense. In extreme emergency, or if my child is under 18 years of age, I understand that Prima Ballet School will attempt to notify the person(s) I have named as my emergency contact(s) of my condition and how to reach me.
Payment Policy
Tuition is charged on a monthly basis, based on equal billing payments by post dated cheques or pre-authorized credit card. Curriculum fees, which varies based on class level, is due at time of registration and is non-refundable. A registration fee per student is due at time of enrolment. This fee is non-refundable. Applicable taxes will be charged to all school fees & products for all programs. A $35 service charge will be applied to NSF and returned cheques. Late payments are subject to a 5% penalty fee, compounded monthly.
A written withdrawal notice must be received by the school administration 14 days prior to the next billing date, otherwise tuition will be forfeited. Any remaining post-dated payments will be cancelled. Once payment has been made for a program, including any tuition payments made in full, that tuition is forfeited. Any post-dated payments will be cancelled. All curriculum fees (costume fees or exam fees) and paid tuitions are non-refundable. In the case of early withdrawal no costumes will be released to the family/students.
Inclement Weather
From time to time classes may be cancelled due to inclement (severe weather conditions) weather. If it is in the interest of the safety of our staff and dancers a class may be cancelled. If a class is cancelled due to bad weather where safety is an issue there will not be a make up class offered.
Taking a Break from Prima:
At Prima we appreciate that sometimes life takes a turn, and you may need to stop dancing for a short time.
You have two choices when deciding to take a break from your dance education:
- “Hold My Spot” We are happy to hold your spot for one month during a 12-month period.
- Withdraw and re-enrol at a later date.
In both cases, written notice is required via our website www.primaballetschool.ca support ticket system. Your notice will be date stamped and kept on file. Phone calls, emails, text messages or social media communications will not be accepted.
Hold My Spot
While taking a break, tuition will be auto charged at 50%. Understand that this is not done retroactively and no refunds or credits will be given.
Withdraw and RE-Enrol
When you withdraw and re-enrol, you will be treated as a new student and required to pay the registration fee again. Prima cannot guarantee your child’s spot in the class.
Exams and Recitals while taking a break:
While taking a break from Prima, there are no guarantees regarding your child’s participation in exams or recital. Once re-commencing classes. participation in exams and recitals are at the sole discretion of the Prima Team and may require private instruction at an additional cost.
Media Release
Prima Ballet School is hereby granted permission to take photographs and videos of the students use for marketing purposes. Permission is also granted to copyright such photography in its name. You must contact the office in person prior to your start date if you would like to opt out of our media release.
Picking Up Your Child
I understand that I must meet my child at the Prima Ballet School's back door for pick up. As soon as your child exits the Prima Ballet School's back/front door, I understand that my child is no longer in the Prima Ballet School's care and his/her safety is entirely my responsibility.
COVID-19
I waive any right to claim against Prima Ballet School for any bodily injury, loss or damage as a result of any exposure to Covid-19. I understand that by signing this waiver, this may affect my legal rights.
Summer Camp Payment Policy
All fees are non-refundable and non-transferable unless a camp needs to be cancelled due to Covid-19 regulations. If you withdraw, your fees are not refunded. Full balance must be paid upon registration. No refunds will be issued unless a camp is cancelled due to Covid-19 or other reasons determined by CDP. No refunds will be issued for any other reason, unless an illness prevents you from participating, in which case a doctor's note will be required. A $25 admin fee applies to all refunds for any reason other than camp cancellation. If a camp does not run (we require a minimum of 5 campers per session), money will be refunded in full.
I understand and agree to the refund policy stated above.
